Physical Therapist (PT) – 4 Day Flexible Schedule
About The Position
Concierge Home Care is hiring a Home Health Physical Therapist (PT). This role offers more control over your schedule, meaningful patient relationships, and the ability to increase income based on productivity. The coverage area is Winter Garden, FL 34787/ West Orange County, FL, with territories designed to minimize drive time through clustered scheduling. The position utilizes a pay-per-visit model with a base rate of $70+ per visit plus Medicare differential pay, mileage reimbursement, and a data plan stipend. Technology such as AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) is provided to reduce documentation time. The company offers structured onboarding, mentorship, preceptorship, ongoing training, free CEUs, and career advancement opportunities. Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Requirements
- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
