About The Position
Concierge Home Care is hiring a Home Health Physical Therapist (PT). This role offers more control over your schedule, meaningful patient relationships, and the ability to increase income based on productivity. The coverage area is Marianna, FL / Jackson County, FL, with territories designed to minimize drive time through clustered scheduling. The position emphasizes flexibility, autonomy, and 1:1 patient care in the home, utilizing AI-assisted charting and a user-friendly EMR to reduce documentation time. Requirements
- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
