About The Position
Concierge Home Care is seeking a Home Health Physical Therapist (PT) to join their team. This role offers more control over your schedule, meaningful patient relationships, and the ability to increase income based on productivity. The coverage area is Vero Beach/Indian River County, with territories designed to minimize drive time through clustered scheduling. The position emphasizes flexibility, autonomy, and technology to reduce documentation time, allowing therapists to focus more on patient care. There are opportunities for growth and clinical support, including structured onboarding, mentorship, ongoing training, free CEUs, and career advancement. The role involves evaluating patients, developing treatment plans, providing therapeutic exercises, educating patients and caregivers, collaborating with physicians, supervising PTAs, and completing documentation.
Requirements
- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
