About The Position

We are seeking a dedicated and compassionate Physical Therapist to join an in-house team of professionals. The role provides person-centered care to help residents achieve positive outcomes and improve mobility, strength, and overall quality of life. The position is listed as full-time/PRN, and the in-house rehabilitation service is non-contracted. New graduates are welcome to apply.

Requirements

• Active and valid Texas Physical Therapist license.
• Good standing with all regulatory agencies and licensing boards.
• Current knowledge of physical therapy treatment practices.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Nice To Haves

• Skilled Nursing Facility (SNF) or long-term care experience.
• Experience with the Net Health Electronic Documentation System.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure treatments are delivered in accordance with the established plan of care.
• Monitor patient responses to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and in a timely manner in compliance with company policies and state and federal regulations.
• Comply with applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.
