About The Position

AdvisaCare is seeking a compassionate Physical Therapist to provide care to patients in their homes. The role focuses on supporting patients’ recovery and independence through personalized, evidence-based rehabilitation and a supportive, patient-centered approach. The position is offered as full-time or PRN, with a flexible schedule and opportunities for professional growth.

Requirements

• Valid, active Physical Therapist license in Michigan.
• Strong communication skills and a friendly demeanor when interacting with patients and families.
• Ability to work independently and manage time effectively.
• Compassionate approach to patient care and desire to learn and grow.
• Valid driver's license and reliable transportation for home visits.

Nice To Haves

• Physical therapy experience, particularly in a home health setting; experience is a plus but not required.

Responsibilities

• Perform thorough patient assessments and develop personalized, evidence-based care plans.
• Guide patients through rehabilitation to help them regain independence.
• Track and document patient progress and achievements.
• Collaborate with an interdisciplinary healthcare team to provide coordinated, holistic care.
• Engage in ongoing learning and professional development.
