Physical Therapist - Raleigh, NC
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Raleigh, North Carolina, and surrounding communities. Care is delivered in patients’ homes and in Senior Living Communities through FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life. The position offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Telehealth and other virtual care may also be incorporated when appropriate. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. The work takes place in a collaborative, interdisciplinary environment with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists. FOX offers full-time, part-time, and PRN / FOX Flex employment pathways. Compensation depends on experience and employment type; benefits are available for eligible roles. H1B sponsorship is available for qualified clinicians, and new graduates may be eligible for the Emerging Professionals Mentor Program.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, assisting with patient transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- May supervise PTA/OTAs, mentor emerging professionals, teach as a Clinical Instructor, or contribute written or social content for additional compensation.
