About The Position
Concierge Home Care is seeking a Home Health Physical Therapist (PT) who desires more control over their schedule, meaningful patient relationships, and the ability to increase income based on productivity. This role offers a flexible schedule, autonomy, and 1:1 patient care in the home. The coverage area includes St. Augustine/St. Johns County, with territories designed to minimize drive time through clustered scheduling. The position utilizes AI-assisted charting and a user-friendly EMR to reduce documentation time, allowing therapists to focus more on patient care.
Requirements
- Active, unencunmbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
