About The Position

Talin Home Health is seeking a dedicated and skilled Physical Therapist to evaluate and treat patients in their homes. The role focuses on improving patients’ physical function and quality of life through individualized care plans tailored to their needs. The Physical Therapist collaborates with other healthcare professionals and provides treatment and support using expertise in medical terminology, anatomy, and patient care. The position is in the Rappahannock area, with other cities available, and work is performed on the road.

Requirements

A degree in Physical Therapy from an accredited institution.
Strong knowledge of medical terminology and anatomy.
Excellent interpersonal skills to build rapport with patients and collaborate with healthcare teams.

Nice To Haves

Proven experience in a clinical setting, particularly in orthopedics or home community care.
Familiarity with discharge planning processes and systems used in patient care.

Responsibilities

Conduct thorough evaluations of patients’ physical conditions and functional abilities.
Develop individualized care plans addressing each patient’s specific needs.
Implement treatment programs using therapeutic techniques such as exercises, manual therapy, and modalities.
Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
Educate patients and their families about rehabilitation, home exercises, and injury prevention.
Collaborate with interdisciplinary teams to ensure comprehensive patient care, including discharge planning for inpatient cases.
Maintain accurate documentation of patient evaluations, treatments, and progress in accordance with clinic policies.
