About The Position

Talin is seeking a dedicated and compassionate Occupational Therapist to provide high-quality care in patients’ homes. The role focuses on improving the functional abilities and quality of life of people with disabilities, injuries, those recovering, and older adults. The therapist develops personalized care plans, conducts assessments, and implements therapeutic interventions. The coverage area is Rappahannock, with other areas available. Work is on the road.

Requirements

Degree in Occupational Therapy from an accredited program.
Valid state licensure as an Occupational Therapist.
Strong knowledge of physiology and occupational health principles.
Excellent communication skills for interacting with patients, families, and interdisciplinary teams.
Ability to develop effective care plans addressing individual patient needs.
Compassionate nature and strong commitment to patient care and rehabilitation outcomes.

Nice To Haves

Experience in case management and working with individuals with disabilities is preferred.
Familiarity with early intervention strategies is a plus.

Responsibilities

Conduct comprehensive evaluations of patients’ physical, emotional, and social needs.
Develop and implement individualized care plans tailored to each patient’s goals and capabilities.
Provide therapeutic activities to improve daily living skills and overall functionality.
Collaborate with healthcare professionals, including speech therapists and case managers, to provide holistic care.
Educate patients and families about treatment options, progress, and ongoing support strategies.
Monitor patient progress and adjust treatment plans based on observed outcomes.
Maintain accurate documentation of assessments, treatment plans, and progress notes in compliance with policies.
