About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in Spring Hill, Tennessee, serving Maury, Williamson, Hickman, and Humphreys counties. The position offers full-time work, flexible scheduling options, independent practice with team support, and opportunities to make a lasting impact on patients and families in their homes.

The Physical Therapist evaluates and treats patients according to physician orders, develops treatment plans and measurable goals, and helps patients reach their highest level of independence and function. The role also includes coordinating with the treatment team, physicians, patients, and families; maintaining timely documentation; and supervising Physical Therapist Assistants and Physical Therapy Aides in accordance with the state practice act.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist licensure in the state.
• American Heart Association CPR certification; certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Physical therapy skills as outlined in the competency checklist.

• Home health experience is preferred.

Responsibilities

• Evaluate and treat patients according to physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and family members, as applicable.
• Document services thoroughly and promptly to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
