About The Position

Residential Home Health and Hospice is hiring a Physical Therapist to provide skilled, patient-centered care to patients in their homes in the Alton area. The role offers autonomy, meaningful patient relationships, interdisciplinary collaboration, and opportunities for career growth. The therapist works independently while receiving support from a field education team and leadership. The position requires travel to patient homes within an assigned territory, including spending approximately 20% of the workday driving. The role is 40 hours per week.

Requirements

• Active Physical Therapist (PT) license in the state where the employee works.
• One year of experience as a Physical Therapist.
• Clinical expertise to deliver quality patient care.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.
• Successful completion of a drug screen prior to employment.

Responsibilities

• Deliver skilled, patient-centered care to patients in a home health setting in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations and functional mobility assessments.
• Develop individualized therapy plans to improve patients’ strength, balance, and independence.
• Educate patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies.
• Think critically and solve problems in real time.
• Complete thorough, compliant EMR documentation.
• Travel to patient homes within the assigned territory.
