About The Position

The Physical Therapist evaluates patients and delivers personalized, evidence-based rehabilitation care to help them regain mobility, independence, and confidence. The role includes assessing functional abilities and developing, implementing, and modifying individualized treatment plans. The therapist works collaboratively with an interdisciplinary healthcare team, supports care transitions and discharge planning, and maintains timely, accurate clinical documentation. The role may involve practice in home care, outpatient, inpatient, skilled nursing, or assisted living settings.

Requirements

Physical Therapist experience required.
Graduate of a Physical Therapy program at an accredited American Physical Therapy Association school.
Bachelor of Science degree or entry-level master's degree.
Active Connecticut Physical Therapist license.
CPR certification required at the time of start.

Responsibilities

Evaluate patients' functional abilities, mobility, strength, balance, coordination, gait, endurance, and pain levels.
Develop individualized treatment plans to maximize movement, safety, and independence.
Deliver evidence-based therapeutic interventions, including exercise, manual techniques, and therapeutic modalities.
Monitor patient progress and modify treatment plans to support optimal outcomes.
Educate patients and caregivers about treatment plans, precautions, equipment, and strategies for continued success.
Collaborate with physicians, nurses, social workers, rehabilitation professionals, and other healthcare team members.
Support transitions of care and discharge planning.
Maintain timely and accurate clinical documentation.
Serve as a mentor and clinical resource for Physical Therapist Assistants, students, and support staff as appropriate.
