Requirements

• Hold and maintain an active license as required by the Arizona State Board of Physical Therapy.
• Have a degree from a CAPTE-accredited program and have passed a state-approved exam.
• Have at least 2 years of experience; home health experience is preferred.
• Hold current CPR certification.
• Have a valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and update individualized care plans.
• Provide physical therapy services according to the physician’s plan of care.
