About The Position

FOX Rehabilitation is seeking licensed Physical Therapists in Scottsdale, Arizona, to provide one-on-one care to older adults in private residences and senior living communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also incorporate telehealth and other virtual care into their schedules.

FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. The position involves collaboration with an interdisciplinary team and educating patients, caregivers, and senior living staff. FOX describes full-time, part-time, and PRN/Fox Flex employment pathways; compensation and benefits vary by employment type. H1B sponsorship is available for qualified clinicians, and new graduates may be eligible for the Emerging Professionals Mentor Program.

Requirements

• Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
