The Physical Therapist will evaluate the clinical needs of the patient, initiate, develop, implement, and revise the patient's plan of care. This role involves addressing all problems within the plan of care and documenting accordingly. The Physical Therapist will participate in inter-disciplinary patient case conferences, evaluate the effectiveness of the established plan of care, and instruct and supervise the Home Health Aide's plan of care every 14 days. This position requires notifying the MD/other involved care providers of significant changes in the patient’s condition, completing admission and other paperwork on a timely basis, and recording clear, concise narrative documentation that reflects skilled care, teaching, direct care, and the patient/caregiver's response. The Physical Therapist must obtain and clarify valid MD orders for all care delivered, plan and coordinate discharges appropriately, inform the patient/MD, and record the patient's disposition in the discharge summary. This role also involves completing all appropriate discharge paperwork and consistently providing service excellence in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence, and service.
