About The Position

Elara Caring is hiring a Physical Therapist to provide home-based care and help patients regain independence, improve mobility, and reach their personal goals. The role includes delivering compassionate, high-quality care to patients in their homes and collaborating with an interdisciplinary team. The Physical Therapist evaluates patients, develops and implements individualized plans of care, assesses outcomes, and maintains accurate clinical documentation. The role may involve directing and supporting therapy assistants and staff, serving as a clinical resource, and promoting physical therapy in the community. It is a physically demanding, fast-paced home-care position that requires travel within the assigned coverage area. Irregular hours or participation in a call rotation may apply. The position offers a $5,000 sign-on bonus. Compensation is determined by qualifications, skills, competencies, and relevant experience.

Requirements

Graduate of an APTA-accredited Physical Therapy program.
Current, unrestricted Physical Therapist license in the state of practice.
At least one year of experience in a healthcare setting.
Willingness and ability to travel up to 50% within the assigned coverage area.
Dependable vehicle, valid driver’s license, and current auto insurance.
Ability to lift, push, pull, and support up to 50 pounds.

Responsibilities

Promote Elara Caring’s mission, philosophy, and policies to support exceptional patient care.
Provide direct patient care, including evaluations, treatment planning, and outcome assessments.
Design and implement individualized plans of care based on each patient’s needs.
Collaborate with interdisciplinary team members to support optimal outcomes.
Direct and support therapy assistants and staff in achieving patient goals.
Maintain accurate and timely documentation, including evaluations, treatment plans, and physician orders.
Serve as a clinical resource and promote physical therapy within the community.
Travel within the assigned coverage area and perform patient lifting, transferring, and positioning as required.
May work irregular hours or participate in a call rotation, if applicable.
