About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Palm Harbor, Florida, and surrounding communities. Clinicians treat patients in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Treatment may also include telehealth and other virtual care when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and support patients’ mobility, independence, wellness, longevity, and quality of life. Clinicians work in a collaborative, interdisciplinary environment and may have opportunities to advance professionally and increase income through referrals, teaching, mentoring, supervising assistants, exceeding patient-care quotas, or creating content. FOX offers full-time, part-time, and PRN/Fox Flex employment pathways. Compensation varies by experience and employment type. Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including exerting medium physical effort; lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; assisting with patient transfers and bed mobility; repositioning patients; standing and walking for extended periods; climbing stairs; and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
