About The Position

Uintah Basin Rehabilitation and Senior Villa is hiring a full-time Physical Therapist to provide resident-centered rehabilitation services at its facility in Roosevelt, Utah. The role involves evaluating residents, developing and implementing individualized treatment plans, improving residents’ mobility and function, and working with residents, families, and the interdisciplinary care team to support resident goals. A sign-on bonus and relocation assistance are available. Pay is based on experience, qualifications, and skills.

Requirements

Current Physical Therapist license in the State of Utah.
Graduate of an accredited Physical Therapy program.
Strong clinical and assessment skills.
Strong communication and interpersonal skills.
Ability to work effectively within an interdisciplinary team.
Ability to maintain accurate documentation and meet regulatory requirements.

Responsibilities

Evaluate residents and participate in therapy screenings.
Develop and implement individualized treatment plans.
Provide physical therapy services to improve mobility and function.
Collaborate with the interdisciplinary care team to achieve resident goals.
Educate residents and families on treatment plans and progress.
Maintain timely and accurate clinical documentation.
