About The Position
Uintah Basin Rehabilitation and Senior Villa is hiring a full-time Physical Therapist in Roosevelt, Utah. The therapist evaluates residents, develops and implements individualized treatment plans, and provides therapy services to improve mobility and function. The role works with an interdisciplinary care team and educates residents and families about treatment plans and progress. The position offers a sign-on bonus and relocation assistance. Pay is based on experience, qualifications, and skills.
Requirements
- Current Physical Therapist license in the State of Utah.
- Graduate of an accredited Physical Therapy program.
- Strong clinical and assessment skills.
- Strong communication and interpersonal skills.
- Ability to work effectively within an interdisciplinary team.
- Ability to maintain accurate documentation and meet regulatory requirements.
Responsibilities
- Evaluate residents and participate in therapy screenings.
- Develop and implement individualized treatment plans.
- Provide physical therapy services to improve mobility and function.
- Collaborate with the interdisciplinary care team to achieve resident goals.
- Educate residents and families on treatment plans and progress.
- Maintain timely and accurate clinical documentation.
