Requirements

• Current Physical Therapist license in the State of Utah
• Graduate of an accredited Physical Therapy program
• Strong clinical and assessment skills
• Strong communication and interpersonal skills
• Ability to work effectively within an interdisciplinary team
• Ability to maintain accurate documentation and meet regulatory requirements

Responsibilities

• Evaluate residents and participate in therapy screenings
• Develop and implement individualized treatment plans
• Provide physical therapy services to improve mobility and function
• Collaborate with the interdisciplinary care team to achieve resident goals
• Educate residents and families on treatment plans and progress
• Maintain timely and accurate clinical documentation
