About The Position

Adventist HealthCare Home Health is seeking a Physical Therapist (PT) for their Silver Spring Territory. The role involves extending God’s care through physical, mental, and spiritual healing. The Physical Therapist will perform initial examinations and evaluations to establish diagnoses, prognoses, and plans of care for patients in various rehabilitation settings. They will provide and direct therapy interventions, involving patients and their families in the care plan, and re-evaluating progress to modify or discontinue services as needed. The role also includes planning for patient discharge, coordinating and documenting all aspects of patient management with the care team, and supervising rehab aides, office assistants, and volunteers. Additionally, the PT will manage their schedule, attend team meetings, communicate patient status, and be responsible for individual professional development and educating others on the benefits of therapy.

Requirements

  • Bachelors, Masters, or Doctorate in Physical Therapy from accredited physical therapy program.
  • 1-year site specific preferred
  • Active Maryland License
  • Active American Heart Association Basic Life Support (BLS) certification required

Responsibilities

  • Perform an initial examination and evaluation to establish an impairment/diagnosis, prognosis and plan of care prior to intervention for all persons served with basic rehabilitation needs in acute care, outpatient or inpatient rehabilitation settings (flexes to any Rehab site as needed).
  • Provide, or direct and supervise, the therapy intervention consistent with the results of the examination, evaluation, diagnosis, prognosis, and plan of care. The therapist involves the patient/client and appropriate others in the planning, implementation, and assessment of the plan of care.
  • Reexamine the patient/client as necessary during an episode of care to evaluate progress or change in patient/client status and modify the plan of care accordingly or discontinue physical therapy services.
  • Plan for discharge of the patient/client taking into consideration achievement of anticipated goals and expected outcomes, and provide for appropriate follow-up or referral.
  • Communicate, coordinate and document all aspects of patient/client management including the results of the initial examination and evaluation, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, changes in patient/client status relative to the interventions, reexamination, and discharge/discontinuation of intervention and other patient/client management activities with members of the care team as indicated.
  • Supervise and direct the patient care activities of assigned rehab aides, office assistants and volunteers.
  • Manage schedule to accommodate range in census and patient load and inform management of any issues with caseload management.
  • Attend team meetings and effectively communicate patient status, using, if needed, the medical record or other treating therapists to ensure sufficient knowledge of the case.
  • Be responsible for individual professional development activities.
  • Educates and provides consultation to consumers, healthcare professionals, students and the public regarding the purposes and benefits of therapy.
