About The Position
Elite Home Health is a Medicare-certified home health agency dedicated to providing compassionate, high-quality care that helps patients regain mobility and independence. They are currently hiring a Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Leesburg and surrounding areas. The agency believes healing happens best at home.
Requirements
- Graduate of an accredited Physical Therapy program
- Current Florida PT license in good standing
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Ability to meet all required background and compliance requirements
Nice To Haves
- Minimum 1 year of PT experience preferred
Responsibilities
- Perform physical therapy evaluations and develop individualized treatment plans
- Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions
- Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices
- Monitor patient progress and adjust treatment plans as needed
- Complete timely and accurate clinical documentation
- Communicate with physicians, nurses, and the interdisciplinary care team
- Supervise PTAs as required
