Physical Therapist - Simpsonville, SC
About The Position
FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Simpsonville, South Carolina, and surrounding areas. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may include in-person visits and, when appropriate, telehealth or other virtual services. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work within a collaborative, interdisciplinary practice. FOX lists full-time, part-time, and PRN/Fox Flex employment pathways. Full-time roles are salaried with benefits; part-time benefits eligibility depends on minimum hours, and PRN/Fox Flex compensation is per unit. Requirements
- Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Potential opportunities include supervising PTAs or OTAs, mentoring emerging professionals, teaching as a Clinical Instructor, and supporting patient care above quota.
