About The Position

FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one care to older adults in Gilbert and Batesburg-Leesville, South Carolina, and surrounding communities. Care is delivered in private residences and senior living settings, with telehealth or other virtual care available when appropriate. Using FOX’s Geriatric House Calls™ model, the clinician helps patients improve mobility, independence, wellness, longevity, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. The clinician works within an interdisciplinary, clinician-operated practice and documents care in FOX’s EMR system in accordance with regulatory standards. This posting describes PRN / FOX Flex work with pay-per-unit compensation. FOX also lists full-time and part-time pathways, but those are separate employment options.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
