About The Position

This is a 13-week assignment for a Physical Therapist (PT) in a Skilled Nursing Facility (SNF). The role involves evaluating and treating patients with a variety of conditions including orthopedic, neurological, and medically complex cases. The primary goal is to develop individualized treatment plans aimed at improving patient mobility, restoring function, and enhancing independence. The position guarantees a 35-hour workweek with 5x8-hour day shifts.

Requirements

• Active Physical Therapist (PT) license in the applicable state.
• Current BLS certification.
• Strong evaluation, treatment planning, and documentation skills.
• Knowledge of Medicare documentation and rehabilitation standards.
• Excellent communication, organization, and interdisciplinary collaboration skills.

Nice To Haves

• Minimum 2 years of Skilled Nursing Facility (SNF) experience.

Responsibilities
