Physical Therapist (Home Care, 1:1) – Duluth, GA – Part-Time
About The Position
Stern at Home Therapy provides outpatient-quality physical therapy to older adults and geriatric patients in a home-based setting. The role offers 1:1 patient care, flexible scheduling, clinical autonomy, and administrative and clinical support. Clinicians can shadow an experienced therapist before committing and receive personalized onboarding, clinical mentorship, and a gradual caseload ramp-up based on their comfort level. Compensation includes tiered per-visit pay of $60–$85 depending on weekly visit volume, with higher tiers unlocking increased rates. Up to two unreplaced cancellations per month are paid. A PRN option offers a higher per-evaluation rate than full-time roles, scheduling based on availability, no required evenings or weekends, a minimum of five patients, and per-visit compensation. Patients are located throughout Duluth, Georgia; coverage zones are designed to minimize travel and support efficient scheduling.
Requirements
- Active Georgia Physical Therapist license.
- Degree from an accredited Physical Therapy program.
- Reliable transportation for home visits.
- Strong communication skills and a patient-centered mindset.

Responsibilities
- Evaluate and treat patients in the home health setting.
- Develop and implement individualized plans of care.
