Physical Therapist Home Care Part Time
About The Position
Corewell Health’s Visiting Nurses Association (VNA), the Home Care Team serving West Michigan, is seeking a licensed Physical Therapist to support patients across Holland, Saugatuck, Fennville, and Hamilton. This home care role offers autonomy to practice at the top of the clinician’s license and help patients regain independence and function in their own homes. Therapists typically see 5–7 patients per day and work within thoughtfully designed geographic territories intended to minimize travel and support work-life balance. The VNA team serves Ottawa County and surrounding communities as part of Corewell Health West. The Physical Therapist examines and evaluates patients with functional limitations and disabilities and develops treatment plans. The therapist also consults with healthcare providers regarding a patient’s continuum of care, appropriately interacts with patients of all ages and cultures, and adheres to Corewell Health customer service standards. The position is part-time, with 20 scheduled hours per week on the day shift. Corewell Health offers a $2,500 sign-on bonus to external hires.
Requirements
- Bachelor’s, master’s, or doctorate in Physical Therapy from an accredited program.
- Current Michigan Physical Therapist License.
- Valid Michigan Driver’s License.
- AHA or ARC Basic Life Support (BLS) certification.
