About The Position
MCH Outpatient Rehab Services seeks a skilled Physical Therapist to provide exceptional, individualized patient care and help patients achieve their goals and return to optimal function. The role is part of a collaborative rehabilitation team that emphasizes teamwork, clinician skill development, and advancement. New graduates and clinicians seeking to develop outpatient skills are welcome to participate in the established mentorship program. The position involves evaluating and treating patients across a range of ages and conditions, developing individualized treatment plans, delivering therapy, monitoring progress, and coordinating care with other healthcare professionals. Candidates may acquire the listed additional competencies through on-the-job training.
Requirements
- Proven experience evaluating and treating patients across a wide range of age groups and conditions; candidates may acquire this competency through on-the-job training.
- Excellent interpersonal and communication skills, including the ability to collaborate effectively with a multidisciplinary team.
- Strong analytical and problem-solving abilities to develop individualized treatment plans.
- A compassionate, patient-centered approach to care.
- Ability to manage time effectively and prioritize tasks in a fast-paced environment.

Responsibilities
- Evaluate patients' physical conditions, medical histories, and goals to develop comprehensive treatment plans.
- Administer therapeutic exercises, manual therapy techniques, and other modalities to improve patients' mobility, strength, and physical function.
- Monitor patients' progress, adjust treatment plans as appropriate, and provide ongoing education and guidance.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate effective patient care.
- Maintain accurate and detailed patient records, including progress notes, treatment plans, and outcomes.
- Stay current with advancements in physical therapy techniques and research.
