This is a fee-for-service position requiring a minimum of 2 shifts per pay period and attendance at a full first week of orientation. If you have no prior CHHA experience, you will commit to training 2 days per week until fully oriented and independently managing.

Requirements

  • New York State PT license
  • Minimum of one year of recent hands on experience as a Physical Therapist.
  • Valid New York State Driver’s License and reliable transportation
  • Work a minimum of 2 shifts per pay period (every 2 weeks)
  • Attend the full first week of orientation, Monday through Friday from 8:30 AM – 4:30 PM

Responsibilities

  • Review patient charts and medical history
  • Develop a personalized treatment plan
  • Teach therapeutic exercises and stretches to improve patient’s conditions
  • Use hands-on therapy to promote healing
  • Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
  • Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload.
  • Provide patient and family education for health promotion as directed by physician’s plan of treatment.
  • Provide care to the most upmost standard
