About The Position
Human Touch Home Health is seeking a licensed Physical Therapist to provide home health services throughout Carbon County, Pennsylvania, with a primary focus on Jim Thorpe, Lehighton, and surrounding communities. This is a flexible per diem opportunity where the therapist can create their own schedule and provide one-on-one care in patients' homes.
Requirements
- Active Pennsylvania Physical Therapist license
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete physical therapy evaluations, reassessments, and discharge planning
- Develop individualized treatment plans and provide skilled therapy services in patients’ homes
- Help patients improve strength, balance, mobility, and independence
- Coordinate care with physicians and the interdisciplinary team
- Complete documentation accurately and on time
