About The Position
Human Touch Home Health is hiring a licensed Physical Therapist to provide home health services in Quakertown, Coopersburg, Upper Bucks County, and surrounding Pennsylvania communities. This is a flexible per diem opportunity where you can create your own schedule and provide one-on-one care in the home setting.
Requirements
- Active Pennsylvania Physical Therapist license
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete physical therapy evaluations, reassessments, and discharge planning
- Develop individualized treatment plans and provide skilled therapy services in patients’ homes
- Help patients improve strength, balance, mobility, and independence
- Coordinate care with physicians and the interdisciplinary team
- Complete documentation accurately and on time
