About The Position
Human Touch Home Health is seeking a licensed Physical Therapist (Per Diem) to provide home health services throughout Fairfax County, Loudoun County, and Prince William County, Virginia. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact with patients one-on-one in their homes.
Requirements
- Active Virginia Physical Therapist license
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete evaluations, treatment plans, reassessments, and discharge planning
- Provide skilled physical therapy services in patients’ homes
- Coordinate care with physicians and the interdisciplinary team
- Manage your own schedule and service area
