About The Position
Make a difference where it matters most — at home. We’re looking for a dedicated Physical Therapist (PT) to join our compassionate home health team. Help patients regain mobility, independence, and quality of life — all from the comfort of their own homes.
Requirements
- Current Physical Therapist (PT) license in Nevada (required).
- Strong clinical, communication, and problem-solving skills.
- Ability to work independently and manage a flexible caseload.
- Valid driver’s license and reliable transportation.
Nice To Haves
- Minimum 1 year of home health physical therapy experience (preferred).
Responsibilities
- Conduct patient assessments, create and update care plans, and establish clear, measurable goals.
- Teach patients and their families exercises and self-care techniques that enhance mobility and promote safety.
- Complete home safety evaluations and provide recommendations for modifications when necessary.
- Work closely with physicians, nurses, and the interdisciplinary team to support high-quality patient outcomes.
- Ensure all visit notes and documentation are completed accurately and on time.
- Join case conferences and regularly update the care team on patient progress.
- Help patients and their families understand care goals, expectations, and ongoing progress.
