Physical Therapist (PT) - Per Visit

Fee-for-Service Physical Therapists are required to: Work a minimum of 2 shifts per pay period (every 2 weeks). Attend the full first week of orientation, Monday through Friday from 8:30 AM – 4:30 PM. If no prior CHHA experience, commit to training 2 days per week until fully oriented and independently managing.
Requirements
- New York State PT license
- Minimum of one year of recent hands on experience as a Physical Therapist.
- Valid New York State Driver’s License and reliable transportation

Responsibilities
- Review patient charts and medical history
- Develop a personalized treatment plan
- Teach therapeutic exercises and stretches to improve patient’s conditions
- Use hands-on therapy to promote healing
- Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
- Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for health promotion as directed by physician’s plan of treatment.
