About The Position

The Physical Therapy Intern focuses on supporting the rehabilitation and functional independence of older adults through supervised clinical practice within the PACE (Program of All-Inclusive Care for the Elderly) program. Responsibilities include assisting licensed physical therapists with patient evaluations, developing and implementing individualized treatment plans, and guiding participants through therapeutic exercises aimed at improving mobility, strength, balance, and overall quality of life. The intern also collaborates with an interdisciplinary care team to address each participant’s comprehensive health needs, while gaining hands-on experience in geriatric care, adaptive techniques, and patient-centered service within a community-based setting. The PACE Program is designed to help older adults remain safely in their homes rather than entering nursing facilities, providing complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies, all coordinated by an interdisciplinary care team. Participants receive personalized care plans and access to a PACE center for social activities, meals, and clinical services.
