At Cherry Blossom PACE, you'll have the opportunity to build ongoing relationships with older adults while working as part of a collaborative interdisciplinary team focused on supporting participants in the community. As a Physical Therapist with Cherry Blossom PACE, you'll play an important role in evaluating functional abilities, developing individualized treatment plans, participating in care planning discussions, and helping participants maintain mobility, safety, and independence.

Requirements

  • Graduate of an accredited Physical Therapy program
  • Current Virginia Physical Therapist license or eligibility for licensure in Virginia
  • Current BLS certification
  • Valid driver's license and reliable transportation
  • At least one (1) year of experience working with a frail or elderly population, or completion of a PACE organization-approved training program.

Nice To Haves

  • Experience in outpatient rehabilitation, home health, skilled nursing, acute care, rehabilitation, or PACE settings
  • Knowledge of mobility training, fall prevention, and functional rehabilitation principles
  • Strong communication and interdisciplinary collaboration skills

Responsibilities

  • Perform physical therapy evaluations, reassessments, and treatment interventions
  • Develop individualized treatment plans and functional goals
  • Provide therapeutic interventions focused on mobility, balance, strength, endurance, pain management, and safety
  • Participate in interdisciplinary care planning meetings and case conferences
  • Conduct home visits as needed to assess safety, mobility, and functional performance within the participant's living environment
  • Recommend adaptive equipment, assistive devices, and environmental modifications
  • Educate participants, caregivers, and team members regarding therapeutic interventions and safety strategies
  • Maintain timely and accurate documentation in accordance with organizational and regulatory requirements
  • Collaborate with the Interdisciplinary Team to support participant-centered care plans
