The Physical Therapist Float is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions. This role involves developing and revising plans of care based on resident needs, establishing functional goals, outcomes, and discharge plans. The position also includes planning for resident discharges and anticipating long-term and/or short-term resident needs. Additionally, the Physical Therapist Float educates other healthcare team members, residents, family, and caregivers on information regarding the care plan for the residents. This role requires providing treatment to all BHS communities as needed and may involve travel to multiple sites in a day.
