About The Position

Diablo Valley Post Acute is seeking a dedicated, compassionate, and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team in a PRN weekend position. This role offers flexible weekend work in a supportive clinical environment where you can make a meaningful difference in residents' lives.

Requirements

  • Current California Physical Therapist Assistant (PTA) license
  • Graduate of an accredited Physical Therapist Assistant program
  • Strong communication, organizational, and interpersonal skills
  • Ability to work independently while collaborating effectively with an interdisciplinary team
  • Commitment to compassionate, resident-centered care

Nice To Haves

  • Skilled nursing, post-acute, or rehabilitation experience preferred

Responsibilities

  • Provide skilled physical therapy interventions under the direction of a licensed Physical Therapist.
  • Assist residents in improving mobility, strength, balance, coordination, and functional abilities.
  • Implement individualized treatment plans and therapeutic activities.
  • Monitor resident response to treatment and communicate progress or concerns to the Physical Therapist.
  • Educate residents and caregivers on safe mobility, exercises, and therapeutic techniques.
  • Collaborate with nursing and other members of the interdisciplinary care team.
  • Maintain accurate and timely clinical documentation.
  • Provide compassionate, resident-centered care focused on achieving meaningful functional outcomes.
