About The Position

All About Home Care, a Houston-based home health care agency serving Houston and surrounding communities since 2004, is seeking Physical Therapist Assistants to care for patients across the Upper Central North coverage area of Greater Houston, mainly including Conroe, Spring, The Woodlands, and surrounding areas. The position is PRN. The agency uses Kinnser software to document patient care.

Requirements

Graduate from a two-year college-level program approved by the American Physical Therapy Association.
Hold a current license in the state of Texas.

Nice To Haves

Two years of home health experience is preferred.

Responsibilities

Understand and adhere to established policies and procedures.
Provide physician-prescribed physical therapy under a plan of care established by a physical therapist.
Improve or minimize patients’ residual physical disabilities.
Help return individuals to an optimum and productive level within their capabilities.
Perform skilled procedures as ordered by a physician and according to the plan of care established by a physical therapist.
Participate with other health care personnel in patient care planning.
Take initial histories and perform initial evaluations.
Consult with a physical therapist regarding changes in treatment.
Instruct patients and family members or significant others in home programs and activities of daily living.
Participate in mandatory in-service programs and present in-service programs as assigned.
Participate in performance improvement activities as assigned.
Attend patient care conferences as scheduled.
Prepare medical records and progress notes, and update care plans for each patient visit in a timely manner, as required by the agency.
