About The Position

OakMed Home Nursing & Rehabilitation is seeking licensed Physical Therapist Assistants to join its talent pool for flexible, home-based opportunities throughout its service area. PTAs provide one-on-one care to automobile accident and workers’ compensation patients, helping them improve mobility, strength, balance, and independence at home. Working under the direction and supervision of a licensed Physical Therapist, the PTA provides treatment visits in the field with support from the clinical team. Cases may be matched to the clinician’s location, availability, and experience; opportunities may suit people seeking to supplement a current position or take on additional work.

Requirements

Graduate of an accredited Physical Therapist Assistant program.
Current, valid, and unrestricted Michigan Physical Therapist Assistant license.
Valid driver’s license and proof of automobile insurance.
Reliable transportation to patient homes.
Strong communication, organization, and time-management skills.
Ability to work independently in the field while following the supervising Physical Therapist’s plan of care.

Responsibilities

Provide physical therapy treatment visits in patients’ homes according to the supervising Physical Therapist’s established plan of care.
Assist patients with therapeutic exercises, mobility training, and functional activities.
Help patients improve strength, mobility, balance, and independence.
Educate patients and caregivers on prescribed exercises, mobility techniques, and safety practices.
Monitor patient responses to treatment and communicate progress or concerns to the supervising Physical Therapist.
Complete accurate and timely visit documentation.
Collaborate with the clinical team to support patient goals.
Follow applicable clinical standards, documentation requirements, and HIPAA regulations.
