About The Position
Concept Rehab is seeking a licensed Physical Therapist Assistant (PTA) to join its rehabilitation team at a Skilled Nursing Facility in Avon, Ohio. The role involves working with older adults in a collaborative post-acute care environment to improve mobility, independence, safety, and quality of life. Concept Rehab is a therapist-owned and therapist-led organization that supports clinicians and emphasizes exceptional, patient-centered care. The position is open to experienced PTAs and new graduates, with support from experienced rehabilitation professionals. Compensation is based on experience, credentials, schedule availability, and market conditions. Requirements
- Graduate of an accredited Physical Therapist Assistant program.
- Current PTA license in the State of Ohio.
- Strong communication and interpersonal skills.
- Commitment to patient-centered care.
- Ability to work collaboratively within an interdisciplinary healthcare team.

Responsibilities
- Provide physical therapy treatments in accordance with individualized care plans.
- Help patients improve mobility, strength, balance, endurance, and functional independence.
- Communicate patient progress and recommendations to the supervising Physical Therapist and interdisciplinary care team.
- Collaborate with nursing staff, physicians, families, and caregivers.
- Participate in care planning and discharge planning activities.
- Educate patients, families, and facility staff on therapy programs and safety strategies.
- Maintain timely and accurate electronic documentation and billing records.
- Assist with restorative and functional maintenance programs.
- Recommend equipment and treatment interventions that support successful patient outcomes.
- Support a positive rehabilitation environment focused on quality care and service excellence.
