About The Position

Elara Caring is hiring a full-time Physical Therapist Assistant to provide personalized home health care to patients in Cleveland, Ohio. The assistant works with patients and caregivers, follows the treatment plan established by the supervising Physical Therapist, and helps patients manage everyday activities, improve mobility and function, and maintain independence at home. Care must align with physician orders, clinical standards, and state practice requirements.

Requirements

• Current, unrestricted PTA license in the state where the employee will work.
• Graduation from an accredited Physical Therapist Assistant program.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation for travel between patient homes and assigned work locations.

Nice To Haves

• Six months or more of PTA experience is preferred.

Responsibilities

• Provide physical therapy services under the treatment plan established by the supervising Physical Therapist.
• Help patients improve mobility, strength, function, and independence.
• Monitor patient progress and communicate changes or concerns to the Physical Therapist.
• Teach patients and caregivers exercises and techniques to support progress between visits.
• Educate patients on safe use of walkers, canes, wheelchairs, and other prescribed assistive devices.
• Deliver care consistent with physician orders, clinical standards, and state practice requirements.
