About The Position
Concord Post Acute is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our dynamic rehabilitation team on a PRN/per-diem basis. This role offers a flexible schedule, competitive pay, and the opportunity to make a real difference in residents’ recovery journeys.
Requirements
- Active California Physical Therapist Assistant (PTA) license in good standing
Responsibilities
- Provide skilled physical therapy treatments under the direction of the supervising Physical Therapist
- Accurately document daily treatments, progress notes, and patient outcomes
- Collaborate with nursing staff, therapists, and interdisciplinary team members regarding resident care plans
- Participate in patient care conferences and ongoing treatment planning
- Assist in training and supporting Restorative Aides to promote continuity of care
- Educate patients, families, and caregivers on maintenance programs and safe discharge planning
- Maintain professional, compassionate communication with residents, families, and staff
