About The Position
Prava is seeking a full-time Physical Therapist Assistant (PTA) to join their team at The Radcliff in Wood Dale, Illinois. The Radcliff is a family-owned senior living community offering Independent Living, Assisted Living, and Memory Care, with a philosophy centered on personalized care, connection, independence, and helping residents live meaningful lives. As a Prava PTA, you will work alongside a Physical Therapist to deliver exceptional, functional therapy to residents, helping them improve strength, balance, mobility, endurance, gait, and functional performance. The role emphasizes building relationships with residents and understanding their individual goals and what matters most to them, rather than just managing a caseload. The PTA will collaborate with an interdisciplinary team and utilize Prava's modern therapy platform with AI-enabled tools for efficient documentation.
Requirements
- A licensed Physical Therapist Assistant in Illinois
- Passionate about working with older adults
- Comfortable working with residents across different functional and cognitive abilities

Responsibilities
- Provide skilled physical therapy interventions under the direction and supervision of a licensed Physical Therapist
- Treat residents across Independent Living, Assisted Living, and Memory Care
- Help residents improve strength, balance, mobility, endurance, gait, and functional performance
- Carry out individualized plans of care with clinical judgment and attention to each resident's goals
- Observe and communicate changes in resident condition, function, and response to treatment
- Reinforce home exercise programs and functional strategies
- Provide caregiver and resident education
- Collaborate with nursing, caregivers, families, activities, and other members of the interdisciplinary team
- Help identify changes in function that may warrant additional clinical assessment
- Participate in fall prevention, mobility, and wellness initiatives
- Document efficiently through Prava's technology platform and AI-enabled tools
- Contribute to a clinical culture focused on outcomes, function, and quality of life
