About The Position

Iconic Care is hiring a Physician Assistant or Nurse Practitioner to provide physiatry care at Skilled Nursing Facilities and similar rehabilitation centers. The clinician will round at 1–3 facilities, typically 1–3 days per week at each location, and focus on patients whose pain or functional limitations interfere with participating in therapy. The position includes focused consulting visits for a set patient load, expected to reach 27–30 patients per day after ramp-up. The clinician will work with substantial autonomy as the sole physiatry provider on-site, with a collaborative clinical team available for consultation. Responsibilities include assessing patients, developing individualized treatment plans with facility primary care teams, delivering targeted interventions, and coordinating referrals when care needs fall outside physiatry. Iconic Care provides hands-on training in facility settings. The role is in person, with a flexible five-day workweek and flexible hours; weekends are optional according to the description, and the schedule section states no weekends, nights, or on-call shifts.
