Physiatry APP (Nurse Practitioner / Physician Assistant)
About The Position
This role involves providing specialized physiatry care in Skilled Nursing Facilities or similar rehabilitation centers. The primary focus is on patients experiencing difficulty participating in therapy due to pain or functional limitations. The APP will round at 1-3 facilities per week, managing a patient load of 25+ individuals daily. This position requires a high level of autonomy, with the APP serving as the sole physiatrist on-site, supported by a collaborative clinical team. The ideal candidate is self-motivated, confident, and passionate about improving rehabilitation outcomes.
Requirements
- Current, unrestricted PA or NP license.
- DEA licensed or willing to obtain DEA licensure.
- Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
- A genuine passion for providing professional, personalized care in an autonomous environment.
- Strong communication skills, with the ability to collaborate with multidisciplinary teams.
- Familiarity with EHR charting.
Nice To Haves
- Nursing home and/or physiatry experience is a plus, but not required.
Responsibilities
- Rounding at 1-3 Skilled Nursing Facilities or similar rehabilitation centers, 1-3 days per week at each location.
- Delivering specialized physiatry care to patients experiencing difficulty participating in therapy due to pain or functional limitations.
- Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams.
- Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management.
- Serving as the sole physiatrist on-site, operating with a high level of autonomy.
- Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.
