About The Position
The Pharmacist is responsible for dispensing prescription medications to hospital inpatients and providing medication information to patients and clinical staff. The role monitors patient health and progress to ensure medications are safe and effective, and adheres to state and federal regulations and Joint Commission standards. The position is at Baptist Health Rehabilitation Hospital, a newly opened 62-bed inpatient rehabilitation facility, and is scheduled PRN.
Requirements
- Graduate of an accredited school of pharmacy.
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
- Current state license to practice pharmacy.
- Excellent oral and written communication and interpersonal skills.
Nice To Haves
- Accredited clinical residency.
- Two years of hospital pharmacy experience.
Responsibilities
- Verify physician medication orders accurately and in a timely manner.
- Monitor and evaluate the six critical processes of medication management, determine risk points, and participate with physicians and clinical staff in continuous quality and performance improvement.
- Verify and approve the work of Pharmacy Technicians.
- Manage and use the computerized pharmacy system.
- Order and store medications to keep them safe and effective.
- Monitor drug therapy for appropriateness and promote and evaluate rational drug therapy to improve pharmaceutical care.
- Perform anticoagulant monitoring.
- Provide information about medications and help patients and clinical staff understand physician instructions.
