The Pharmacist Resident will monitor daily IV to PO's, renal dosage adjustments, and place work on the pharmacy intervention program for DOP review. This role generates a high number of cost-saving interventions and contributes articles for the pharmacy newsletter. The resident will monitor microbiology cultures and antibiotic usage daily, as well as pharmacokinetics, recommending appropriate level draws and dosage adjustments. Additionally, they will monitor Drug Use Evaluations daily, making recommendations when medications are used outside established criteria, and document this information. The resident will contribute drug monographs and/or drug reviews for the Pharmacy and Therapeutics Committee, including cost-efficacy analysis. They will also provide nutrition support therapy with the nutrition support team, recording patient duration and indication for TPN, and reporting to quarterly meetings of the Nutrition Support Service.
