Requirements

  • Licensed Pharmacist
  • Experience in oncology pharmacy
  • Ability to assess medication therapy regimens
  • Ability to manage medication therapy and supportive care
  • Ability to educate patients on medication use
  • Ability to review chemotherapy orders
  • Ability to provide drug information and optimize drug therapy
  • Ability to participate in collaborative and protocol-driven practices
  • Ability to support the development, updates, and creation of new drug monographs
  • Ability to present summaries of diagnosis-related group updates
  • Experience in education and mentorship of pharmacy students and residents

Responsibilities

  • Provide comprehensive direct patient care to patients within the medical oncology team.
  • Deliver medication management services in coordination with other healthcare professionals.
  • Assess medication therapy regimens.
  • Manage medication therapy and supportive care.
  • Educate patients on medication use.
  • Review chemotherapy orders.
  • Provide drug information and optimize drug therapy.
  • Participate in collaborative and protocol-driven practices as appropriate.
  • Support the development, updates, and creation of new drug monographs for oncology diagnosis-related group guidelines.
  • Present quarterly summaries of diagnosis-related group updates at Oncology P&T subcommittee meetings.
  • Be involved in the education and mentorship of pharmacy students and residents.
