About The Position

Accurate Home Care is hiring a Personal Care Assistant (PCA)/Community First Services and Supports (CFSS) Support Worker to provide direct patient care and help patients with daily living activities. Under the supervision of a Qualified Professional/Supervising Professional, the Support Worker follows each patient’s care plan to help maintain their health status and independence. The role is based in Apple Valley, Minnesota, and is available full-time or part-time at $18.00 per hour. Accurate Home Care provides services intended to help individuals remain at home while they recover. Requirements

Demonstrate sensitivity and compassion toward clients.
High school diploma or GED preferred, but not required.
Be at least 16 years of age.
Have a valid driver’s license and reliable vehicle.
Be fluent in English.
Be able to lift up to 50 pounds.
Have a PCA/CFSS Certificate; applicants may ask their recruiter.

Responsibilities

Provide assistance according to the patient’s care plan.
Communicate appropriately with the Clinical Manager and scheduling team.
Assist patients with daily activities, including bathing, feeding, grooming, and dressing.
Perform light housework or other activities related to independent living.
Report changes or concerns to the Clinical Manager.
